Abstract

The incidence of pulmonary TB has increased every year in Indonesia, it is estimated that the number of Tuberculosis cases was 425,089 cases in 2017, the highest number of cases occurred in the province of West Java with 78,698 cases and in the city of Sukabumi reached 754 cases. One of the problems experienced by pulmonary TB patients is breathing pattern disorders, so that one of the actions that can overcome breathing pattern disorders is by adjusting the position of Orthopnea. The purpose of applying orthopnea is to overcome breathing pattern disorders. case study method with observation and interview techniques. Pulmonary TB is an infectious disease mainly caused by the bacterium Mycobacterium tuberculosis which can attack various organs of the lungs. Positioning Orthopnea is an action that places the patient in a 90o position sitting on the bed with a table and stacking pillows on the bed to lean on. The procedure in giving the orthopnea position is to adjust the client's position, put a pillow on the table and then make sure the client's body is leaning forward on the table that has been given a pillow, this action is carried out for 3-5 minutes. The case study conducted on Mrs. P for 3 days the client data found that he was short of breath, blood pressure 100/90 mmHg, pulse 90 x/minute, temperature 36.5 0C, and respiration rate 27x/minute. Interventions carried out are monitoring vital signs, giving orthopnea position to the client, then monitoring respiration rate and O2. The results of the action showed a change before the orthopnea position the client's respiratory rate was 27x/minute and before the orthopnea position the client's respiration rate was 22x/minute. The results of the case study showed a change in the respiratory rate in pulmonary TB patients after the orthopnea position. The orthopnea position can be applied to pulmonary TB patients with impaired breathing patterns that affect the respiratory rate.
